(WHAS11) - Carissa Foushee of Payneville, Kentucky died Tuesday from injuries sustained in an accident Monday where her vehicle collided with another on an icy highway.

Foushee's two children 2-year-old Adison and 6-year-old Michael were pronounced dead at the scene.

In the accident, a sport-utility vehicle crossed over the center line on Kentucky 144 near Payneville and crashed head-on into a truck around 7:30 a.m. Monday.  The driver of the other vehicle, David Barr,  had non-life-threatening injuries.

Police say the roadway was slick and ice covered at the time of the accident and was a contributing factor. All individuals involved were properly restrained in their vehicles.

The accident is being investigated by the Kentucky State Police.
