(WHAS11) -- Five months after a daycare employee was killed and 14 children were injured in a van crash, WHAS I-team took a closer look at daycares which had been cited by the state for serious safety violations.
 
Our investigation led to one daycare losing it's transportation license, after we caught critical safety violations of tape during a surprise visit to the facility.
 
We also discovered a convicted felon working at a daycare, who was involved in an accident with children in a van she didn't report to the state.
 
The I-team also learned that despite the Commonwealth issuing sanctions against troubled chid care centers, safety issues are often ignored until children are hurt.
 
WHAS's Adam Walser hit the streets and asked the tough questions on behalf of tens of thousands of parents whose children are enrolled in local daycares.
