LOUISVILLE, Ky. (AP) -- A preliminary report on the crash of a medical helicopter in southeastern Kentucky says witnesses gave conflicting accounts of weather conditions at the time.

Three crewmembers died when the aircraft crashed last week in the parking lot of an elementary school outside Manchester. There was no patient on board.

The report by the National Transportation Safety Board says several eyewitnesses indicated the weather was clear and the helicopter was spinning prior to crashing.

The report says other witnesses reported dense fog in the area at the time of the crash.

The helicopter burst into a fireball after crashing.

The report sets out circumstances of the crash but doesn't delve into the cause.

NTSB officials say it generally takes about a year to report the cause of a fatal aircraft crash.
