Panel begins reviewing child death cases

FRANKFORT, Ky. (AP) — An independent panel appointed by Kentucky Gov. Steve Beshear has begun reviewing child deaths and severe abuse or neglect cases.

Among the cases the panel reviewed on Monday was that of 3-year-old Alayna Adair of Christian County. According to media reports, Department of Community Based Services Commissioner Teresa James told the panel that there were "egregious" failures that led up to the girl's death in 2011. She said the case isn't representative of how most reports of abuse and neglect are handled.

The case was one of four reviewed.

The panel is tasked with making recommendations to improve investigations of child abuse cases.
