INDIANAPOLIS (AP) -- IndyCar has announced the results of its investigation into the death of driver Dan Wheldon from a crash during a race in Las Vegas in October. The report indicates that Wheldon was killed when his head hit a post in the fencing at Las Vegas Motor Speedway. Although the contact with the post killed Wheldon, the investigation determined several factors contributed to what became a "perfect storm."
