FRANKFORT, Ky. (AP) -- Kentucky's child-protection agency is overhauling the way it reviews the deaths of children from abuse or neglect.

The Lexington Herald-Leader reports the changes at the Cabinet for Health and Family Services come after a December analysis by the newspaper that found inconsistency among the reviews, which are required in cases where state social workers were involved with the families.

The paper found that some workers issued detailed recommendations about what could have been done better to prevent the death. Other workers turned in single-page reviews that lacked basic information. In a few cases no reviews were completed.

Teresa James, acting commissioner of the Department for Community Based Services, said the agency now has a standard form for reviews, and recommendations will require action plans to ensure they are followed.
